2023 Evaluation Forms

The 2023 Evaluation Forms are ready for download on the Forms page of the district website. Please note that there is no longer a separate form for Associate pastors. All pastors should complete the Self Evaluation Form.

• 2023 Evaluation Form: Pastor Self Evaluation

• 2023 Evaluation: PPRC Assessment - To be completed by each church/charge PPR for each pastor appointed to the church. The Pastor is NOT to fill in the PPR form.

The deadline to submit the completed and signed forms to the district office is Monday, May 1. The district will not accept any incomplete and/or unsigned forms. Please make sure your form(s) are fully complete before submitting them. You can email the completed form to Becky Hoener, fax it to (636) 891-9700 or snail mail it to the office (514 Earth City Expressway, St. Louis, MO 63045).

Please Note

• All Churches receiving a new pastor this summer do not need to fill out a PPRC Evaluation form. 

• All Pastors who are retiring, moving out of, or moving into new appointments in the district do not need to fill out a Self Evaluation form. 

• Lay Persons supplying churches on an assigned basis should complete a Self Evaluation form. This applies only when 1 lay person is assigned to the church. This does not apply to a rotation of lay persons serving a church. This is a change from previous years.

• Churches who have one Lay Person assigned should complete a PPRC Evaluation form. This applies only when 1 lay person is assigned to the church. This does not apply to a rotation of lay persons serving a church. This is a change from previous years.

The schedule for clergy evaluation meetings is not finalized yet. As soon as that is ready, the Sign Up will be published and publicized. As a reminder, if we do not receive both the Self Evaluation Form and the PPRC Evaluation by the May 1 deadline, you will be removed from your meeting time slot and rescheduled at the DS's convenience.
